I, {fname} {lname} understand and agree that All Road Communications is authorized to immediately bill my credit card xxxx-xxxx-xxxx-xxxx for the Amount of ${monthly_amount} for the rental of Garmin satellite equipment and service.

I understand and agree that prior to my trip, I will test the device to ensure I know how to use it and that it is working properly. I also understand that for quality assurance, an ARC representative will be available to help me set up and use the device if assistance is needed. I understand that I waive my right to any refunds for any orders not tested prior to use.

I understand and agree that a satellite equipment must have a clear and Interrupted line of sight to the sky in order to work.

I understand and agree that my rental equipment will be shipped to {address1}, {address2} {city}, {state}, {zip}, {country}

I understand and agree that my rental equipment will arrive no later that {start_date}.

I understand and agree that my rental equipment will arrive back at All Road Communications located at 2222 4th Ave Suit A. San Diego, CA 92101, no later that {stop_date}. Should the rental equipment be returned after this date, then daily late fees will apply of $4.28 per day. 

I understand and agree that a $50 cancellation fee will apply should I cancel this rental prior to my scheduled pickup date or before the equipment is shipped. I further understand and agree that no refunds will be issued once the rental equipment has been picked up from our location or shipped by All Road Communications. If I choose to return or cancel the rental after taking possession or after the order has dispatched, I remain responsible for the full rental charge.

I understand and agree that satellite communication is not a perfect science and as such, from time to time, I may experience service interruption. I also understand and agree that I will be liable to pay for all calls, sms, or data made on the satellite rental equipment no matter how short.

I understand and agree that if the satellite equipment is lost or stolen I am liable to pay for the cost of the used satellite phone in the amount of $1,000.00 to be charged to my credit card on file for the rental. 

As the credit card holder, I also authorize All Road Communications to charge my credit card for airtime charges incurred during the period of the rental above and beyond the original minute bundle I have selected at $1.00 per minute of overage and for any damages/loss of equipment up to 90 days following the conclusion of the rental period.
